When: January 31
Time: Jan 31 10:00 A.M. - 5:00 P.M. | NEW DATE ADDED: Feb. 6 7:30 A.M. - 12:00 P.M.
Where: 45145 W. Madison Avenue, at City Hall
How to make an appointment: Please contact M.O.M. at: 480/967-3767 or 800/285-0272 to schedule your convenient appointment. Please have your insurance card available.
Day of appointment: Please bring the following with you to your appointment: your insurance card, your doctor's name and address, and the location of where you had your prior mammogram films.
Cost: Insurance coverage with UnitedHealthcare and other select carriers.  Non-insured patients: $167.00

 

It is your responsibility to consult your individual plan for coverage and limitations. You will be responsible for any charges that are not covered. Patients under age 30 must provide a doctor's referral. Baseline mammograms recommended at age 35-39, (unless family history or symptomatic) and most insurance plans cover women every year over 40 years old.

M.O.M. uses Full Field Digital Mammography and CAD reading and also accomodate breast cancer implant patients.
