American Passport

The U.S. Department of State has designated the City of Maricopa as a Passport Acceptance Facility.

Applying for the First Time

  • You must apply in person and bring the following items with you:
  • (DS-11) Application (also available at city hall) Complete in black ink.
  • Proof of U.S. Citizenship (one of the following). *Please note proof of citizenship need to be submitted with the application and will be returned once the passport is processed.
    • Previous U.S. Passport
    • Certified birth certificate issued by the city, county or state
    • Consular Report of Birth Abroad or Certification of Birth
    • Naturalization Certificate/Certificate of Citizenship
  • Picture Identification
    • Previous U.S. Passport
    • Current Naturalization Certificate
    • Certificate of Citizenship
    • Current, valid Driver's License, Military ID Card, City, State, or Federal Government ID Card
  • One Passport Photo
    • Passport photos can be taken locally at CVS, Walgreens or UPS Store.

Under Age 16

  • The minor must appear in person. All applications for children under 16 require the consent of both parents/legal guardian(s).

Age 16 to 17

  • The minor must appear in person. For security reasons, parental consent may be requested. If a child does not have identification of his/her own, a parent must accompany the child, present identification, and cosign the application. Adult fees apply.

*Passport fees can be made payable to the Department of State (check or money order only) per application.
**Execution fees are made payable to the City of Maricopa (check or cash only).

Note: The City of Maricopa does not accept debit, credit cards or starter checks.

Renewing Your Passport

Standard Service for Passport Delivery is currently 6-8 weeks. 

Expedited Service for Passport Delivery is 2-3 weeks. 

  • Additional $60 per application
  • Overnight delivery is available for an additional fee
