Before scheduling your appointment, please take a moment to review the following information.
Applying for a Passport
Before scheduling an appointment confirm you meet the requirements to submit FORM DS-11:
- (DS-11) Application complete in black ink. Forms are available on-site but applicants must arrive early to have form filled out by appointment time.
- Proof of U.S. Citizenship (Original/Certified Copy and an additional photocopy):
- US Birth Certificate issued by City, County or State
- Consular Report of Birth Abroad or Certification of Birth
- Naturalization Certificate/Certificate of Citizenship
- Full validity, undamaged US Passport
- Please note your original proof of citizenship needs to be submitted with the application and will be mailed back once the passport is processed.
- Picture Identification (and photocopy of the front and back of the ID):
- Fully valid Driver's License
- U.S. Passport must be undamaged
- Current Naturalization Certificate/Certificate of Citizenship
- Government Employee ID
- Note: you will be asked to present secondary ID when presenting any of these document:
- Out-of-state Driver's License
- In-state, fully valid learner's permit with photo
- In-state, fully valid non-driver ID with photo, or
- Temporary driver's license with photo
- One Passport Photo
- Passport photos are not available on-site, but can be taken locally at CVS, Walgreens or UPS Store.

Under Age 16
- Minor must appear in person with parents/legal guardians. All applications for children under 16 require the consent of both parents/legal guardian(s). You cannot renew a child’s passport with form DS-82.
- If one or both parents cannot apply in person with the minor, you will need to show additional documentation such as the Statement of Consent Form DS-3053. The form must be notarized and presented at the time of the appointment with a copy of the ID provided to the notary (front and back).

Age 16 to 17
- The minor must appear in person. Must show one parent/legal guardian is aware minor is applying. If a child does not have identification of his/her own, a parent must accompany the child, present identification, and cosign the application. Adult fees apply.

Appointments are Required for All Applicants
Due to high demand, we have limited availability. Appointments are only available one (1) week in advance. If you do you not see an available appointment, please check back the following day at approximately 5 a.m.

Passport Renewal
Check Renew my Passport by Mail (state.gov), or Renew my Passport Online (state.gov) for eligibility to renew by mail or online with Form DS-82, you must meet the following requirements:
- Must be able to submit most recent passport without any damage.
- Previous passport was never reported lost/stolen.
- It was issued within 15 years.
- It was issued when applicant was over 16 years.
- You cannot renew your child’s passport using form DS-82. If the applicant is a minor or was a minor at the time of issuance of the previous passport, they must appear in person and submit form DS-11.
- Applicant’s name is the same on the previous passport or applicant can provide evidence such as marriage certificate or divorce decree.
- It was not issued with limited validity.
If one or more of the requirements are not met, applicant must schedule an appointment and submit form DS-11. We do not process renewals if you meet the requirements of Form DS-82, you must submit by mail or online and follow the forms directions.

Payment Instructions: It's important to note there are two (2) separate transactions
- The Application Fee is paid directly to the US Department of State:
- Must be check, money order or cashiers check only.
- The Execution Fee is paid directly to the City of Maricopa:
- Cash, check, and card except starter checks.

Processing Times
- Routine: 4-6 weeks
- Expedited: 2-3 weeks and an extra $60
Consider the total time it will take to get a passport when you are booking travel. Processing times only include the time your application is at a passport agency or center.
- It may take up to 2 weeks for applications to arrive at a passport agency or center. It may take up to 2 weeks for you to receive a completed passport after we print it.
